Before you start

Consent forms are PDFs you upload. Each one is versioned, and every signature is tied to the exact revision the patient signed. You'll need a role with EMR access.

Step 1: Upload a form

  1. In the side menu, under EMR, select Consent Forms.
  2. Select New form.
  3. Select Select a file and choose your PDF (PDFs only).
  4. Enter a Form Name and select Create.

Step 2: Keep it current

  1. Each form shows its revision number. Select it to open the PDF.
  2. To update the wording, open the form's menu (three dots), select Edit, then Add revision and upload the new PDF. Select Update.
  3. To retire a form, select Archive form.

Renaming a form doesn't create a new revision. Past signatures stay tied to the revision that was signed.

Step 3: Attach it to a service

  1. In the side menu, select Procedures and open the service.
  2. Select the pencil.
  3. Choose the form under Consent Form, and save.

How patients sign

At the visit, the provider opens the appointment in the NomadMD app and taps Consent Form. The patient draws their signature on the phone, and the provider is recorded as witness, with the time and location (see Charting & Completing Visits). Appointments needing a signature show Signature Required in the app.

Signed copies

Signed forms appear under Signed Consent Forms on the patient's profile and in Chart Notes. Each signed copy adds a certificate page showing who signed, when, and where.
