Before you start
A form template is a chart your providers fill in. There are three types:
- Assessment Form: completed before treatment (history, screening questions).
- Intervention Form: what was given during the visit.
- Intake Form: questions the patient answers before the visit (see Intake Forms).
You'll need a role with EMR access. The EMR menu only appears if your organization has an EMR set up.
Step 1: Open templates
In the side menu, under EMR, select Form Templates. Each template shows its type.
Step 2: Create a template
- Select Create Form Template.
- Pick the type in the top corner: Assessment Form, Intervention Form or Intake Form.
- Enter a Name.
Step 3: Add fields
- Use the Add bar: Text Field, Dropdown, Multi-Select, Yes/No, Time Selection or Header (a section title). Intake forms also offer File Upload.
- Type each field's label. Text fields can show placeholder text. For dropdowns, add each choice with Add option.
- Drag the handle on the left to reorder. Select X to remove a field.
- Optional (assessment and intervention only): choose a Parent form template to show this template as a section inside another.
- Select Create.
Step 4: Attach it to a service
- In the side menu, select Procedures and open the service.
- The details show its Assessment Form, Intervention Form, Intake Forms and Consent Form.
- Select the pencil, choose the templates, and save.
Visits booked after that use the new chart. When a visit has several services, their forms are combined into one assessment and one intervention.
Tips
- A service's medication protocol is added to its chart automatically, with the doses filled in.
- Keep templates short. Providers chart on the go.
